I. COMMITMENT TO YOUR PRIVACY
Prestige Psychology Suite is dedicated to protecting the privacy and confidentiality of your Protected Health Information (PHI) in compliance with the Health Insurance Portability and Accountability Act (HIPAA) and Georgia privacy laws.
PHI includes any identifiable health information related to your mental health condition, treatment, or care. This Notice of Privacy Practices ("Notice") explains how your PHI may be used, disclosed, and your rights regarding its protection. Please review it carefully and contact us with any questions.
II. OUR LEGAL DUTY TO SAFEGUARD YOUR PHI
By federal and Georgia law, we are required to:

✔ Provide this notice to inform you of our privacy practices.
✔ Follow the terms of this notice until it is revised or updated.
This notice applies to all records created or maintained by Prestige Psychology Suite. We reserve the right to update this notice, and the latest version will always be available in our office or upon request.
III. HOW WE MAY USE AND DISCLOSE YOUR PHI
We will not use or disclose your PHI without written authorization, except in the following cases:
1. For Treatment
We may share PHI with healthcare providers involved in your care.
📌 Example: If you also see a psychiatrist, we may share the necessary information to coordinate your treatment.
2. For Health Care Operations
We may use PHI to improve services, manage our practice, and conduct quality assessments.
📌 Example: Reviewing cases for training and quality control.
3. For Payment
We may use PHI to bill and collect payment for our services.
📌 Example: Providing information to your insurance company for reimbursement.
4. Employees and Business Associates
We may share PHI with employees or business associates who provide administrative, billing, or IT support.

Cookies
If you leave a comment on our site you may opt-in to saving your name, email address and website in cookies. These are for your convenience so that you do not have to fill in your details again when you leave another comment. These cookies will last for one year.

In certain cases, we may use or disclose PHI without your consent:

As required by Georgia state law, court orders, or subpoenas.
To prevent fraud, abuse, or healthcare violations.
2. Public Health & Safety
To report child abuse, elder abuse, or threats of harm to self or others.

3. Health Oversight Activities
For audits, licensing, or regulatory reviews by government agencies.
4. Serious Threat to Health or Safety
If disclosure is necessary to prevent immediate harm.
5. Minors' Privacy Rights (Georgia Law)
Georgia law allows minors (age 12 and older) to consent to outpatient mental health treatment without parental involvement, except under certain conditions.
6. Workers' Compensation
To process claims related to work-related injuries.
7. Research & Education
PHI may be used for research purposes if fully de-identified or with your consent.
8. Bill for your services
We can use and share your health information to bill and get payment from health plans or other entities.
V. YOUR RIGHTS REGARDING YOUR PHI
You have the right to:
✔ Request Copies of Your PHI – You may obtain copies of your records upon written request.
✔ Request Restrictions – You may ask us to limit certain disclosures, but we are not legally required to agree.
✔ Confidential Communications – You may request PHI be sent to an alternate address (e.g., work instead of home).
✔ Request Amendments – You may request corrections to inaccurate PHI.
✔ Receive an Accounting of Disclosures – A list of when and with whom we have shared your PHI.
✔ File a Complaint – If you believe your privacy rights were violated.

Changes to the Terms of this Notice
We can change the terms of this notice, and the changes will apply to all information we have about you. The new notice will be available upon request, in our office, and on our website.
VII. PRESTIGE PSYCHOLOGY SUITE’S RESPONSIBILITIES
✔ We are required by law to maintain the privacy and security of your PHI.
✔ We will notify you promptly if a data breach compromises your PHI.
✔ We will not use or share your information for marketing or sales purposes without your written consent.
By signing our “Consent to Treatment” form, you acknowledge that you have read and understood this Notice of Privacy Practices.
